New Starter Forms
- All staff should complete the Staff Record of Acceptance form in order to confirm agreement with terms and conditions of the contract and return to their HR Business services coordinator.

- If a student at the university and being offered work (e.g tutors) then a confidentiality form should be completed and returned

- If not in possession of a P45 from a previous employer then a new starter checklist should be completed an returned.

- A personal details and Equal opportunities form should be completed and returned

- A SD65 form should be completed for BSMS staff who meet the criteria for an NHS pension
- A declaration form for any staff wishing to bring materials and equipment into the university (science areas) should be completed and returned.
